ADMISSIONS PROCESS

APPLICATION PROCEDURES

The following information must be sent to Bailey Military Institute for review from our Admissions Department.

  • An application with a $25 non-refundable application fee must be filled out and signed by the custodial parent and student.

  • A report of medical history must be filled out and signed by the custodial parent.

  • A statement of waiver must be filled out and signed by the student and custodial parent.

  • A complete transcript of grades with achievements or I.Q. test results.

  • Two teacher evaluations from the students English and Math/Science teachers from the last school attended.

  • Standards for students must be read and signed by the student and custodial parent.

  • A phone or virtual (Skype) interview with the applicant is required before acceptance. Please call 832-778-9632 or email baileymilitary@att.net to set this up.

  • Upon acceptance, a $100 non-refundable tuition deposit is required. This deposit will go toward the cost of tuition.

ACCEPTANCE PROCEDURES

​As part of the registration process parents/guardians (and in some cases students) must sign and return the following forms prior to or during their registration process. Completed forms may be sent by mail to BMI to the attention of Welcome Packet Coordinator, 2626 South Loop West, Suite 402, Houston, TX 77054. You may also choose to fax the documents to 832-778-9632 or you may scan and email them to baileymilitary@att.net with the subject heading of Welcome Packet for Student's Name.

  • Authorization to Dispense/Monitor and Transport (Medical)

  • Health and Immunization Requirements

  • Hold Harmless and Authorization for Medical Treatment

  • Pre-Participation Medical History

  • BMI Application for Automobile Driving/Riding Privileges

  • TAPPS Acknowledgement of Rules

  • Copy of front and back of your medical insurance card. This will be critical if medical treatment is needed.
